A couple more reasons might be due to physical problems.
First, your child's hearing or vision may be compromised in some way.
Second, he or she may be dyslexic or be struggling with any number of other "learning" challenges.
Be careful, though!  
Many children are diagnosed by their teacher prematurely and really need a parent and doctor assessment.
Whatever the case, the parent is usually the best person for the job of discovering and treating learning disabilities. 
This article addresses ways you can go about determining  where the biggest hurdles are.
